trip – first to register and pay will fill the spots.

We are tentatively scheduled to fly out early Friday morning, June 26th
on American Airlines and return late night Friday, July 3rd. We will give flight information
at a later date.

CONTRIBUTIONS/PAYMENTS
Donations are tax deductible and can be made through the Managed Mission
website. A link is available below for family and friends making a donation to your account. Ask them to find your name in the drop down
box via the link (after your registration and deposit has been approved.)
PASSPORT- see www.travel.state.gov for passport information.
*If you already have a passport, check
your expiration date.
If it expires in
2020, RENEW IT.
*Be sure to update your profile on
Managed Missions with your passport information when you have it.
*Provide
us a copy of your passport-that will be kept for the trip.
*Make sure to sign your passport on the
upper top page before making a copy for us.
SHOTS
See your doctor for necessary shots you might
need. Envision recommends Typhoid and an
up-to-date Tetanus shot. There are no
required shots to travel to the Dominican Republic.

NOTARIZED PARENTAL FORM
Students, under the age of 18, traveling internationally without a
parent, needs to have a form completed/notarized.

painting, cleaning up etc.
